WAIVER AND RELEASE:
By registering as a participant, I agree to hold harmless the Dayton Autism Society, its officers and parent organization, the Autism Society of America, as well as the Payne Recreation Center, the city of Moraine, Ohio and its employees from any injury or illness sustained by participating in the Dayton Autism Society 5K Walk/Run for Autism and Resource Fair event activities. By agreeing to the Waiver and Release, I give permission for the Dayton Autism Society to release my photo and or name to the media for publicity purposes only. I also agree to allow my likeness to appear in photos posted to the website in either Individual/Team photos or various walk day photos that may be posted to the website or in printed media used by the Dayton Autism Society.
